Original and copy of the secondary or higher education certificate with the transcript
Certificate of the Unified National Testing (UNT) results
Medical certificate (form No. 075) + chest X-ray image
Certificate of educational grant award (if available)
Eight 3x4 cm photographs
Copy of the identity card
Copy of the vaccination card (form No. 063) (for full-time students)
Copy of the military registration certificate or military ID (for male full-time students)
